Dr. Allen Momongan
Dr. Allen MomonganDr. Allen Momongan, DDS is a graduate of the University of Illinois where he received degrees in both economics and biology. After receiving his D.D.S. from Northwestern University in 1994, he completed a General Practice Residency program at Rush University Medical Center in Chicago, IL where he regularly treated medically compromised patients. There he was trained in aspects of plastic, ENT (ear, nose, throat), and oral maxillofacial surgery as well as emergency room care and anesthesiology.

Dr. Momongan utilizes the latest technology available including CaesySM educational software, air abrasion technology, cosmetic imaging software, digital x-rays and computer and video digital enhancement equipment. He has an passionate interest in aesthetic dentistry. To create a natural look, Dr. Momongan develops cosmetic solutions that are in harmony and blend with the rest of a patient's features. Working exclusively with a local lab in downtown Chicago, he partners with technicians to customize the color of his veneers and crowns for his patients.

He also stays on top of the latest techniques in dentistry by completing over 30 hours per year of continuing education in his field.

Dr. Momongan is a member of the American Dental Association, Illinois Dental Association, Chicago Dental Society, and Academy of General Dentistry. He is on the Associates Board of Rush University Medical Center. He is an avid sports fan, and member of the Chicago Sport and Social Club. Dr. Momongan is married, and has one daughter.

 

Dr. Matthew Flak
Dr. Matthew FlakDr. Matthew Flak, DDS grew up in North Canton, Ohio. After completing a degree in Microbiology from Miami University in Oxford, Ohio, Dr. Flak continued on to dental school at the Ohio State University where he earned his DDS degree in June, 2001. Dr. Flak practiced general dentistry in Columbus, Ohio for two years before moving to Chicago in February, 2003. Dr. Flak is a certified Invisalign® provider and enjoys creating beautiful smiles with the use of clear braces. He has trained under an accredited cosmetic dentist as well as an implant specialist and loves to use the skills he learned to place porcelain veneers, crowns and white fillings to achieve a wonderful smile or just restore teeth to their healthiest. He is dedicated to the care of his patients and enjoys studying all the new techniques and materials that dentistry has to offer in order to provide his patients the best care possible. Dr. Flak enjoys cycling, golfing, reading, and exploring the many neighborhoods that surround Chicago.

 

Tricia Ciak, RDH
Tricia Ciak, RDHTricia Ciak, RDH, is a 2002 graduate of Kalamazoo Valley College in Kalamazoo, Michigan. There she completed her A.A.S. in Dental Hygiene and graduated with honors. Tricia moved to Chicago in September of 2002 and began employment with the Dental Practice of Lincoln Park and Dr. Momongan. In her spare time, Tricia enjoys running, traveling, spending time with her family, water and snow sports and going to the theatre.

 

 

 

Sara Hultman, RDH
Sara Hultman, RDHSara Hultman, RDH, is a 2003 graduate of Milwaukee Area Technical College in Milwaukee, Wisconsin. There she completed her A.A.S.in Dental Hygiene and graduated with honors. Sara is a current and active member of the Sigma Phi Alpha Dental Hygiene Honor Society. Sara moved to Chicago in 2003 and later began her employment with the Dental Practice of Lincoln Park and Dr. Momongan. In her spare time, Sara enjoys working out, traveling, and spending time with her family.

 

Kayla McDougall, RDH
Kayla McDougall, RDHKayla McDougall, RDH, is a 2005 graduate of Ferris State University in Big Rapids, Michigan. There she completed her A.A.S. in dental hygiene and graduated with honors. Kayla moved to Chicago in September 2005 and later started working with the Dental Practice of Lincoln Park and Dr. Momongan. Kayla is an active member of the Illinois State Dental Society. In her spare time, Kayla enjoys snowboarding, volleyball, and any other outdoor activity.

 

 

 

Eve Shira, RDH
Eve Shira, RDHEve Shira, RDH, has a Bachelors of Science in Health Care Management and Dental Hygiene. Her experience of 15-plus years has given her a well-rounded background in periodontics, cosmetics and general dentistry. Her interests include travel, theater, opera, skiing and literature. She is currently working for Dr. Matthew Flak.

 

 

 

 

 

Laura Catrambone, RDH
Laura Catrambone, RDHLaura Catrambone, RDH, is a 2006 graduate of Harper College in Palatine, IL. There she completed her A.A.S. in Dental Hygiene. Prior to getting her dental hygiene degree, Laura received her Bachelors degree in Communications from the University of Illinois Urbana-Champaign. Laura has been employed with the Dental Practice of Lincoln Park since July 2006 and is currently working for Dr. Matthew Flak. She enjoys traveling, playing sports, and spending time with her family.

 

 

Jamie Stalf, RDH
Jamie Stalf, RDHJamie Stalf, RDH, graduated with honors from the Dental Hygiene Program at Parkland College, Champaign, Illinois in 2001. She continued her education at Illinois State University earning a Bachelors of Science in Health Education and Nutrition. In addition to providing care for her patients, Jamie works with dental hygiene students at UIC College of Dentistry in the clinical setting. She is a member of the American Dental Hygienists' Association and certified in administering local anesthesia and sedation dentistry. Jamie is dedicated to educating her patients and helping improve their overall oral health and wellness. She enjoys spending time with family and swimming, running, and weight lifting. Jamie works for Dr. Flak.
